Original WWII Dutch NSB member questionnaire with photo

This completed NSB questionnaire with passport photograph is in good condition and comes from a collaborator who was born in the Dutch East Indies.
The man was a member of the NSB, Waffen-SS, Jeugdstorm and attended the Jugendführerschule in Potsdam.
Then from 1942 he attained the rank of Hopman and was assigned to permanent service in the Jeugdstorm.
Within the NSB, he participated in the speaking course, formation course and resilience course.
He was also a bearer of the Jeugdstorm 5-year honorary badge.
Completed questionnaires are very scarce and rare, unique to the collaboration collection!

Originele WWII Nederlandse NSB ingevulde vragenlijst 

Deze ingevulde NSB vragenlijst met pasfoto is in goede conditie en is afkomstig van een collaborateur welke geboren was in Nederlandsch-Indië.
De man was lid van de NSB, Waffen-SS, Jeugdstorm en ging naar de Jugendführerschule in Potsdam.
Daarna vanaf 1942 behaalde hij de rang hopman en werd hij in vaste dienst aangesteld bij de Jeugdstorm.
Binnen de NSB deed hij mee aan de sprekerscursus, vormingscursus en weerbaarheidscursus.
Hij was tevens drager van het Jeugdstorm 5 jaren eeredraagteken.
Ingevulde vragenlijsten zijn zeer schaars en zeldzaam, uniek voor de collaboratie verzameling!
