Original WWII Polish ‘Legitymacja Ubezpieczeniowa’, a social insurance identification card issued in Lublin.
The identification card belonged to Leon A. Felczyński, born on 22 July 1902 in Puławy.
The document bears number 1380754 and was issued on 4 July 1940 by the ‘Ubezpieczalnia Społeczna w Lublinie’, the Social Insurance Institution in Lublin.
